Heightened self-focused attention and judgment biases concerning the probability and cost of negative social events are important cognitive factors maintaining social anxiety. The aim of this study was to examine how self-focused attention and estimated probability and cost of negative social events would change over the course of cognitive behavioral group therapy (CBGT) and how these changes would influence social anxiety symptoms. Forty seven participants with social anxiety disorder received CBGT and completed questionnaires measuring self-focused attention and probability and cost estimates of negative social events. As hypothesized, self-focused attention and probability and cost biases reduced significantly during the treatment. Hierarchical regression analysis indicated that change in self-focused attention was a significant predictor of symptom reduction. Reductions in estimated probability and cost of negative social events were also significant predictors of treatment gain, even after controlling for the effect of self-focused attention.
